If you wish to have someone else speak to us regarding your application or WFTC account information, you will need to provide us with written approval using our form: Confidential Tax Information Authorization (CTIA).
About the CTIA form
The Confidential Tax Information Authorization form authorizes us (DOR) to share your confidential tax information with a third party. It can also allow us to send your confidential information using regular (unsecure) fax or email. You can specify whether you want to share ALL of your tax information or limit it to a certain time period or category.
How to submit the CTIA form
You can submit your CTIA form through the following:
- Attach it to your Working Families Tax Credit application.
- Upload it to your MyDOR account and send it via secure message.
- Turn it in to a local DOR office.
- Can I authorize a friend or family member to speak on my behalf?

Yes. You can authorize anyone you choose.
Note: If you include your spouse on your WFTC application, they are part of your account and are authorized to speak to us.
- Can I give a verbal authorization over the phone or in-person?

No. Without a CTIA, we cannot speak to anyone about your tax information.
